Dallas Middle School recently issued the following announcement.
Physicals: Fall Sports
Fall Sports Physicals for DHS and DMS students are scheduled for the following dates:
Monday, July 26, 2021 - 8:00 AM (Cheerleading, Football, Golf, Tennis)
Friday, July 30, 2021 - 8:00 AM (Field Hockey, Soccer)
Friday, August 6, 2021 - 1:00 PM (Cross Country, Volleyball, INCLUDING NEW MIDDLE SCHOOL INTERMURAL VOLLEYBALL)
ALL PHYSICALS WILL TAKE PLACE AT DALLAS HIGH SCHOOL. IF YOU ARE NOT ABLE TO ATTEND THE SCHEDULED DATE FOR YOUR SPORT, PLEASE ATTEND ONE OF THE OTHER DATES LISTED ABOVE.
Prior to coming for your physical, please print out and complete the PIAA physical form found HERE. Ensure all signatures for parents and students have been completed, along with the information requested on Sections 1-6. If you are not able to print a copy of the physical form, you may pick up a copy at DHS, DMS, or the Athletic Office.
School sports physicals provided by the Dallas School District are free of charge. Current School Board Policy requires students to have a new physical completed for each new athletic season.
The first day of mandatory practice for fall sports is August 16, 2021.
No student will be permitted to practice without an approved physical.
